Appetite Suppressant Program

Appetite suppressant programs are an integral part of Urban Skin Solutions weight loss approach. Drugs and injections that reduce appetite are used on a short-term basis to clinically treat obesity and promote weight loss. You will meet with our nurse practitioner for a medical exam and history review. The medical assistant will also privately weigh you, administer your Lipo B12 and fat burning injection, take your measurements if desired, and calculate your BMI. After your medical exam and history review the nurse practioner will determine if you are a candidate for Phentermine, a prescription appetite suppressant. Common reasons why someone wouldn’t qualify for the
prescription is a BMI below 27, high blood pressure, or a history of heart problems. If you are a candidate she will write you a 35-day prescription. We require that you return on a monthly basis to meet with the nurse practitioner for a physical exam, diet and exercise review, and also to refill your 35-day appetite suppressant prescription. If you only want to do the weight loss injections you only need meet with the nurse practioner for a check-up every 90 days.

Urban Skin Solutions Cancellation Policy

**To reschedule or cancel an appointment a 24 cancellation notice must be given.**

Urban Skin Solutions does provide a courtesy reminder call the day before the scheduled appointment. Therefore, we ask that you extend the same courtesy by providing 24 hours notice for a cancellation. Also if you are more than 15 minutes late to your scheduled appointment we are not required to see you and the appointment is documented as a no show. When a client does not show for their scheduled appointment or is late, we do not have the opportunity to fill that time slot with another client, causing our business to lose money.

The fees are as follows:

1st offense: no charge
2nd offense and More: $25

For those patients who have package purchases the above fees still apply, if you do not comply with the above fees then one service will be automatically deducted from your package.

**After your first complimentary missed appointment we require that your credit card information is held on file in order to make a future appointment**

For All Appointments 40 min or longer we require a credit card must be given at the time the appointment is made. An Automatic $25 charge will be charged to the card if you fail to give 24 hours notice to cancel or reschedule the appointment
